Proposed Discharge Planning Change

Nursing's role in discharge planning is wide ranging. We monitor for potential complications with treatment as well as evaluate the patient's ability to understand their disease process and care for themselves when discharged home. While in a care setting we are evaluating our patients response to illness and aiming to lessen the impact that illness will have on their life. We have a heightened insight on what care plans will be carried out by the patients and often offer recommendations for adjustments. Nurses evaluating patient's health literacy is essential to the success of this process.

The Centers for Medicare and Medicaid Services ( CMS) hopes to revise the discharge planning process at facilities who participate in Medicare and Medicaid program. Your voice should be included in this process.
